Currently, therapeutic management focuses on resuscitation and immediate treatment with antivirals, immune modulators, and cytokine targeted therapies to dampen the overly exuberant immune response. However, early anecdotal evidence of post-COVID morbidity and mortality, the submerged part of the metaphoric iceberg, suggests an urgent need for and focus on specialized aftercare.
Older people and people with many serious medical conditions are the most likely to experience lingering COVID-19 symptoms, but even young, otherwise healthy people can feel unwell for weeks to months after infection. Common signs and symptoms that linger over time include:

Organs that are affected by COVID-19 include.

The same may be true for people who have had COVID-19. Post-COVID symptoms like lung fibrosis, vasculitis, etc. are a result of a phenomena called “cytokine storms”, which could be defined as an excessive and uncontrolled reaction against the pathogen.

Unfortunately, the devastating effect of this excessive response is considered as the major complication of COVID-19, even late after the onset of the infection.

As matter of fact the use of Resveratrol in Kawasaki’s Disease has been evaluated by Huang et al in 2017. Kawasaki Syndrome has been witnessed in pediatric COVID-19 patients. They concluded that Resveratrol may modulate the inflammatory response of coronary artery in Kawasaki’s Disease and explore the role of autophagy in the pathogenesis and alternative therapy for coronary arterial lesions in Kawasaki’s Disease.
Unfortunately, the low bioavailability of Resveratrol limits its potential use. Therefore, to obtain maximum benefits from resveratrol it is obvious that liposomal formulations should be preferred.

Metil Grupları tek bir Karbon atomuyla karakterize olmaları sebebiyle Metilasyon reaksiyonlarına yaygın olarak tek karbon döngüsü de denilmektedir.

Metil Reaksiyonları yaşamın en temel molekülleri olması nedeniyle tek karbon döngüsü tek başına sağlıklı bir hayat sürmesinin önkoşulu gibi karşımıza çıkmaktadır. Metilasyon reaksiyonları yaşamın sürdürülebilir olması açısından aşağıdaki reaksiyonlarda etkin rollere sahiptir:
- Hücre Çeperlerinin sentezlenmesi
- DNA sentezi
- Serotonin, Adrenalin, Dopamin Sentezi
Tüm bu yukarıdaki reaksiyonların kusursuz bir şeklide işlemesi için vücudun temel metil donörü olan S-adenozil-l-metiyonin (SAMe) sentezi kusursuz işlemelidir.

Ancak ne yazık ki geçirilen mutasyonlar nedeniyle bu Yolaktaki en öneli reaksiyonlardan bir tanesi olan Homosistein-Metiyonin döngüsü bozulmuştur.

Bu insanlar maalesef en önemli metil donörü olan SAMe’yi üretememektedirler. SAMe’nn uygun miktarda üretilememesi halinde daha önceden de ifade ettiğimiz üzere:
- Sağlıklı Hücre bölünmesi meydana gelmez, hücrelerin sağlıklarını korumaları için Fosfolipitlere ihtiyaç duyar. Uygun miktarda Metil grubu yoksa hücre membranları meydana gelemez.
- DNA sentezi, DNA kendini kopyalarken metil gruplarına ihtiyaç duyar. Yeterli miktarda metil grubunun olmaması düzgün DNA bölünmesini bozabilir.
- Serotonin, Adrenalin ve Dopamin gibi Nörotransmitterler sadece metil gruplarının varlığında uygun miktarlarda senztelenebilir.
- Vücudumuzun en güçlü antioksidanı olan Glutatyon da sadece SAMe varlığında sentezlenenbilmektedir.
Elbette bu kadar önemli fonksiyonlara sahip SAMe’nin üretilememesinin bazı klinik sonuçları da olacaktır.
Son yıllarda yapılan araştırmalar Kanda Homosistein seviyeleri yüksek olan hastaların kalp krizi, inme v.b. kardiyolojik hastalıklara daha yatkın olduğunu göstermektedir.
Tedaviye Dirençli veya Majör Depresif bozuklukları olan hastalarda SAMe sentezinin bozuk olduğunu gösteren çalışmalar bulunmaktadır.
Karaciğer yağlanması olup ve tek karbon döngüsü bozuk hastalarda Karaciğer Yağlanması daha kötü bir seyir izlemektedir.
SAMe metabolizması bozuk olan erkeklerde Sperm kalitesinin düşük olduğunu gösteren klinik çalışmalar bulunmaktadır.
SAMe döngüsünün bozulması Kadınlarda Polikistik Over Sendromunun gelişmesinde rol oynadığını gösteren klinik çalışmalar da bulunmaktadır.
Görüldüğü üzere tek Karbon döngüsü bozukluğu ve SAMe’nin yeteri kadar üretilememesinin son derece ciddi önemli sonuçları vardır.
Bu nedenden dolayı Genetik olarak mutasyona sahip olan inşalar veya kanda homosistein seviyeleri yüksek olan kişiler kesinlikle sadece Metilfolat ve Metilkobalamin almalıdır. Zira bu kişilerde diğer formları tam anlamda faydaları olamayacaktır.
Ancak son zamanlarda takviye edici gıda olarak SAMe içeren ürünler de piyasaya verilmiştir. Bu kişilerin özel ihtiyaçlarına uygun olacak SAMe içeren ürünler ile ilgili önümüzdeki günlerde bloğumuzda yeni yazılara yer verilecektir.
